GENERAL

What is Evolve Personal Health?
We are a membership-based healthcare practice. We do not accept insurance for our services, as we work outside of the scope of traditional medicine. Our practice is limited in the number of members we accept in order for us to maximize services and time given to each client.
How is Evolve different from other practices?
Typical medical practices are built upon the traditional paradigm - patients see their doctor only when they are sick. However, our approach at Evolve focuses on preventative care. Through coaching, we develop personal relationships with our members to improve their physical, nutritional, and introspective well-being.

We also offer unique services such as yoga and meditation classes, as well as community events.
I've been hearing about your tailored programs. Tell me about them.
In addition to our general membership plan, we will also be launching tailored programs for specific health conditions like hypertension, diabetes, and high cholesterol. These progams are separate from our membership plan in that they include a pre-determined amount of appointments, protocols, and exercises to see measurable improvements. Please call us to learn more about these defined, effective programs that we hope to launch in Fall 2017!
How long are consultations and visits?
It is up to you! In-office visits can be scheduled up to an hour. Telemedicine consultations are typically 15-30 minutes in duration.
What is telemedicine? How does it work at Evolve?
Telemedicine is the remote delivery of healthcare services. At Evolve, you can have a secure video consultation with any member of your health team through your computer or phone.

What is Introspective Coaching?
This is a unique service offered only at Evolve. We believe our mental, emotional, spiritual, and social experiences form our perceptions. Our Introspective coaching addresses these aspects through a blend of life coaching and meaningful conversations. We seek to provide greater clarity through self-discovery, philosophies of meditation, and mindfulness-based stress reduction.
What is VimChi?
VimChi is a methodology that merges the inner and outer aspects of one’s being to achieve optimal growth. It stems from the founders of Evolve - Vim, a physician, and Chitra, a life coach with a background in meditation. The literal meaning of Vim is positive energy or enthusiasm, and Chi means life force or life breath. VimChi is our proprietary method of health assessment.
Is there a minimum or maximum number of a la carte services when purchasing under the Maintenance Plan?
Not at all. A la carte services can be purchased at any time as a member of the Maintenance Plan. This access plan is intended for you to use as you see fit. Please contact us to learn more!
Do you have a massage therapist on staff?
We do not have a massage therapist as part of our staff. However, we do host a company, Renew You Tampa, that provides massage services at our location. This allows our members the opportunity to receive a much discounted rate when they book massages through Renew You Tampa. We encourage our members to take advantage of their services, as we highly encourage massage for its well-studied therapeutic benefits.
What classes do you offer? Are they for members only?
We currently offer yoga, breath work, and meditation classes for members and non-members. Regular classes are free for members. However, workshops and special classes may have a cost associated. Please remember to view the cost before you RSVP.

INSURANCE

Do you accept health insurance?
We do not accept any form of insurance, including Medicare, for physician or coaching services.

JOINING

How do I join?
You can sign up on our website or call us to join. Ideally, you can stop by, giving us the opportunity to meet you in person and answer any questions you may have.
Is it mandatory to purchase a Day at Evolve before joining?
It is not mandatory, but it is highly encouraged. This will give us an opportunity to get to know each other and enable you to really envision how Evolve can fit into your daily life. You can sign up on our website or call us to join.
Is there a minimum term?
There is a minimum membership term of three months for our general membership program.
Are there cancellation fees?
The first 90 days are non-refundable. Afterwards, there are no cancellation fees. However, please keep in mind that all cancellation requests must be made at least 10 days before your next billing cycle or you may be billed.

COMMUNICATION

How do I schedule my appointments?
Members will be able to make appointment requests through our Patient Portal or by calling us. We will send you a confirmation by email or phone. Appointments can be made for in-office doctor visits and counselor sessions, as well as phone and telemedicine sessions. House calls must be made by speaking with us directly.
Tell me more about telemedicine
Members will be able to have video consultations with the Evolve team. When it is time for your appointment, you will log into our virtual "waiting room" using our HIPAA compliant app or from your browser and attend your video consultation.
